The ExoDx Prostate test, or “EPI,” is a simple, non-invasive urine test that can help you and your doctor determine the need for a prostate biopsy. The ExoDx Prostate test provides actionable information that aids the prostate biopsy decision for men in the PSA ‘gray zone’ of 2-10ng/mL, and is not dependent on PSA or other clinical risk factors.

The Insurance Process by ExoCARES

  1. Submit a claim
    A claim is submitted to your insurer.
  2. Receive an EOB
    Receive an Explanation of Benefits (EOB) from our insurer. This is not a bill.
  3. Payment covered by your insurance
    In many cases insurance will pay for the complete cost of your test.
  4. ExoCARES coverage & appeal
    In the event your insurance company does not pay, we may appeal on your behalf.
  5. Personalized consultation
    If, after appeal, there is no insurance coverage, an ExoCARES Representative will contact you to review options. One call will be made, please call back.

What exactly is PSA?

Prostate-specific antigen, or “PSA,” is a routine blood test measured for many men in the primary care or urology setting. While PSA levels are not prostate cancer-specific, it is the standard of care to indicate prostate cancer diagnosis that may lead to further testing or a prostate biopsy. Making the biopsy decision is an important consideration between you and your doctor because there are sometimes physical complications. The ExoDx Prostate test will provide your doctor with important information to help you make this decision.

Move forward with confidence as interpreting the score is easy. The ExoDx Prostate test score ranges from 0–100. If your score is above 15.6, there is a higher risk for clinically significant prostate cancer that needs further investigation, such as a prostate biopsy. Talk to your doctor about this procedure.

Frequently Asked Questions

Is the EPI Test covered by health insurance?

Most claims are covered by insurance companies and we continue to add coverage from additional insurance plans. However, only your insurance company can determine if the cost of the test is a covered benefit. The code to use is CPT PLA 0005U.


If I have health insurance, will I have any out-of-pocket expense?

Given the differences among health insurance programs that determine covered services, co-insurance, co-pay and deductibles, it is difficult for Exosome Diagnostics to determine what any one individual’s out-of-pocket cost for the test might be at any given time in the year.


If I have gone through the appeals process, but am still unable to pay, what are my options?

Please review the information on our Patient Assistance Program (PAP) to determine whether you qualify for a reduced out-of-pocket cost for The EPI Test. In addition, several payment plan options are available. Please contact our billing department at 844-EXOSOME (844-396-7663), Option 3, for more information.


What is the cost of the test?

The cost of the test depends on your individual health insurance coverage (for example, in-network, out-of-network, co-insurance, deductible and co-pay)


Do you have a flexible payment plan?

Yes, we offer an interest-free payment plan over a period of up to 6 months.


How do I make a payment?

Please call our billing department at 844-EXOSOME, Option 3, to arrange for payments. Screening tests are eligible for reimbursement with a flexible spending account (FSA), a health savings account (HSA), a health reimbursement arrangement (HRA), or a limited care flexible spending account (LCFSA).

What if my insurance plan sends me a check directly for The EPI Test?

  1. Endorse the check by signing the back and writing the following under your signature: "Payable to Exosome Diagnostics, Inc."
